As for crazy adults who want meat-grinder replacement genitalia, ON THEIR OWN DIME, I don't care what they do so long as they 1) don't ask others to pay for it and 2) don't get any special rights. Have at it. Maybe George Foreman will invent a Trans-grinder slice-'n-dice kit and sell it on QVC.
